Enlarged Prostate – An Overview

It is interesting to note that the aging dog (like his master) is also prone to prostatic enlargement, but (unlike his master) does not often develop retention of urine. However, due to the bulging upwards of the enlarged prostate the dog’s rectum is compressed. This engenders a feeling of continuous fullness in the rectum and induces straining. The prostates of eunuchs are small and underdeveloped, and enlargement of the prostate is unknown.

Benign prostatic hyperplasia (BPH) is one of the most common diseases to affect men beyond the middle age. The prevalence goes up with age, and thus the total number of patients is increasing as a result of aging population. A majority of these people suffer gradual progression of symptoms and the concomitant discomfort, and require either medical or surgical treatment. The growth and development of the prostate is under the influence of male hormone “testosterone”.

Rather surprisingly, there is no close correlation between the size of the prostate and the extent of outflow obstruction. However, the larger the prostate, the greater is the risk of BPH complications such as acute urinary retention and the need for surgery.

BPH is usually a slowly progressive condition. An average increase in the prostate is 1-2 cm per year. BPH is the most common condition affecting the prostate accounting for over 80 per cent of prostate disease. The defined risk factor for BPH is age. Clinical BPH seems to run in families.

In the early stages of the disease, the patient complains of hesitancy, a reduced stream and incomplete bladder emptying and then frequency, urgency and nocturia. Later on, prolonged micturition, acute urinary retention, urge incontinence, etc, can have a negative effect on the quality of life and may be associated with sexual dysfunction like erectile dysfunction and disorders of ejaculation.

The treatment of BPH can be with the help of drugs or by going in for surgery. Some of the drugs used are Finasteride, Duatasteride, Terazosin, Alfuzosin and Tamsulosin. Three Top Herbs Used in Menopause

In many industrialized countries, menopause can be a time of great discomfort and stress. Menopause is not a disease, to be viewed as something to be ‘treated’, but a passage in life from childbearing years. The symptoms American women may experience include hot flashes, mood swings, irregular bleeding, memory problems, decreased sexual interest and other changes that effect the enjoyment of sex. Although dietary habits and amount of activity a woman engages in can decrease many of these problems, genetics and previous health can also play a role in how severe the symptoms can be for a woman.

Multiple studies today indicate that taking hormone replacement therapy (HRT) may not be as safe as previously believed. As just one example, in July, 2002, the Women’s Health Initiative Study published results that demonstrated that the risks of HRT outweighed the benefits. Now more and more women entering menopause are considering ‘natural’ solutions to resolving uncomfortable symptoms of this life transition.

There are three particular herbs of interest when entering menopause. They include Black Cohosh, Chaste Tree Berry and Red Clover.

Black Cohosh

Black Cohosh is also known as Black Snakeroot, Bugwort, Rattleweed, Snakeroot, among other names. Black Cohosh was first used medicinally by Native Americans who introduced it to European colonists. Do not confuse black Cohosh with two unrelated plants, Blue Cohosh and White Cohosh.

Black Cohosh is one of the most popular herbal remedies for menopausal symptoms. Overall, the research supports that using Black Cohosh can help reduce menopause-related hot flashes. The research is a bit mixed on exactly how Black Cohosh works and if it has estrogen-like activity. Some research does show that in any estrogen-like activity, this activity does not pertain to the reproductive organs, such as breast, endometrium, and ovaries. A big question for women going into menopause is whether this is a safe herb for women with a history of cancer; the research generally says it is. However, for all women, this herb should not be taken for unlimited amounts of time.

There are also concerns of liver toxicity with the use of Black Cohosh. Until more is known, anyone who wishes to take Black Cohosh should consult their health practitioner and monitor their liver function to be on the safe side. It would also be wise to avoid taking it along with other herbs that may increase the risk of liver damage, which include androstenedione, chaparral, comfrey, DHEA, germander, kava, niacin, pennyroyal oil, red yeast, and others.

Remifemin, a branded black Cohosh product, has been used in Germany since the mid-1950s to manage menopause. Research that has been done generally has used either Remifemin or two other products produced by Enzymatic Therapy and Phytopharmica.

As mentioned earlier, before taking this and most other supplements, make sure to discuss them with your doctor, particularly if you are taking any prescription medications or have any medical conditions. All herbs do have various interactions with other substances.

Chaste Tree Berry

Chaste Tree Berry is also known as Vitex, Chinese Vitex, Hemp Tree, and Mang Jing Zi, among others. An interesting bit of history is that historians say that monks chewed Chaste Tree parts to make it easier to maintain their celibacy.

This herb has been most often recommended for relieving symptoms related to premenstrual syndrome (PMS), such as bloating, headaches and cramping. Although this is where most of the research has focused, it is now being studied to see if it can help with menopausal symptoms as well, and has also become quite popular for this use although there have only been three studies looking into how it could help with hot flashes in menopause.

The therapeutic effects of Chaste Tree Berry have primarily been attributed to its indirect effects on various neurotransmitters and hormones. It seems to increase levels of progesterone and decrease levels of estrogen. Theoretically, Chaste Tree Berry might interfere with the action of dopamine antagonists such as antipsychotic drugs due to dopaminergic effects of Chaste Tree Berry. It is believed that this herb could interfere with any hormone-related medications. Finally, there is a report that some women who were receiving invitro fertilization procedures and taking Chaste Tree Berry were unable to get pregnant. Consequently, if you are taking any medications or have a history of any medical conditions, speak to your health professional before starting this herb.

This is an herb that is not recommended for women with a history of any hormone-related cancers. However, for others it does appear to be safe. So, particularly for women who suffer from PMS or have mild menopausal-related hot flashes without a history of cancer, this could be an effective herbal remedy.

Red Clover

Other names for Red Clover include Genistein, Meadow Clover, Purple Clover, Trifolium, among others. Do not confuse Red Clover with Sweet Clover, Soy or Liverwort, however.

As a food, Red Clover is considered safe. Medicinally, there have only been four studies on Red Clover for menopause and three of these showed no benefit. Research has considered a possible blood cholesterol-lowering affect, decreasing prostrate size, and perhaps a positive effect on cancer, but research has not definitively supported any of these benefits, yet.

There is some evidence that red clover isoflavones can inhibit oxidative and conjugative metabolism. The list of possible drug and herbal interactions is so large that the best recommendation is to never consider this herb without a long discussion with your doctor, or avoid this herb completely.

This is an herb that appears to be safest when eaten as opposed to taken as a nutrition supplement. Although it could be safe for healthy persons with no medical history or on medications or other herbs, it does not appear to relieve any menopause symptoms. It also is an herb recommended to be avoided by any woman with a history of cancer.

We have just reviewed three popular herbs taken to relieve menopause symptoms. Of the three, the one with the best safety and effectiveness profile is Black Cohosh. Chaste Tree Berry has been shown to be most effective for PMS and should not be taken by women with a history of breast cancer. Red Clover appears to be something safer to eat than take as a supplement and should be avoided by women with a history of cancer, as well as people on any other medication or herbal product. Something that I hope readers were able to realize is that although herbs are a natural form of medication, they still do work like drugs and should be taken as seriously as prescription medications are. Interactions are possible with all substances and before taking any herbal product, I highly recommend that you consult with a healthcare professional with adequate education and background to help you make an informed decision on how to best handle symptoms.

Finally, it’s important to point out that in some societies there is not even a word for menopause. Women pass through this stage of life without notice. This makes it very clear that lifestyle has much to do with how a woman experiences this period of time in her life. If you currently suffer from symptoms of menopause, there are several things that you should evaluate. These include:

The amounts of fruits, vegetable and whole grains you eat daily,

Your intake of saturated fat, animal products and refined foods,

Your alcohol intake and smoking habits,

Regular exercise habits.

Is Men’s Skin Care Problematic Today?

So, what do you say, do you think men face problems in skin care products? I would say “yes”. As with any market that proves itself profitable, most companies have recognized this trend, and are quick to throw out their new product to quickly cash-in on this new trend. Are they throwing out their products too fast? I think allot of them are. Over the past several years there has definitely been a sizeable increase in the sales of men’s skin care products. Now, men have about the same opportunity to waste their money on unnecessary and problematic skin care products as women do.

Think about this, As a man, while shaving, so you experience razor burn? Do you think its the way that you shave that’s giving you razor burn? Honestly, you do what you con to save your face from wounds.

Take it from me, as a man, I shave a lot. I use the same products that my dad used. I experienced razor burn. I thought it was the way I did it. To figure out what was really going on, I had to find out what razor burn really is. Razor burn is a series of ingrown hairs. Most shaving creams contain irritating ingredients such as alcohol, menthol mint, and camphor, as well as high levels of potassium or sodium hydroxide. These skin irritants make the hair follicle and skin swell, forcing the hair up and away from the skin, supposedly allowing for a closer shave. Unfortunately, the irritation and resulting swelling cause some of the hair to be hidden by the swollen follicle and skin. Note that sodium hydroxide is used in many skin care products at very low levels(the lesser amount in the ingredient list on the label) to adjust the ph value. In these low quantities it is not irritating to skin. Additionally the swollen skin makes it harder for the stubble to find its way back out. If hair begins to grow ( and it almost immediately does) before the swelling is reduced, the likelihood of ingrown hairs are increased.

Depression and Erectile Function – Psychology of Sexuality

A recent research showed that the brain activity is totally different during sexual activity. That is why men with depression have difficulty in having satisfactory life. The serotonin level in depressed patients is lower than normal adults.

If the depression is treated with right medicines, their erectile function will also improve significantly. But the class of medications known as SSRI antidepressants are used to treat depression in men. But unfortunately these drugs also cause erectile dysfunction in 65% of men taking SSRI for depression.

Depression and ED are highly inter related. If a man is mentally depressed, he will have erectile dysfunction and similarly a man who has erectile dysfunction because of some other medical condition may have depression. So if a depressed man taking drugs for that will eventually have ED.

The recent research shows that in men undergoing treatment for depression, the new class of drugs called PDE5 has improved their erectile function. So in turn it paves a way to decrease their depression too. So with PDE5 class drugs both erectile function and symptoms of depression improved. Viagra, levitra, cialis belongs to the PDE5 class of drugs.

Another study reveals that underlying cardiovascular disease may also lead to erectile dysfunction. Since the cardiac output is low in such patients, the penile blood flow will be a bit lower than normal even though they are sexually aroused. The usage of PDE5 class drugs had significantly lower blood pressure and heart rate, improvement in measures of oxygen consumption and carbon dioxide production.

Definition of Premature Ejaculation:

Most men have experienced the problem of premature ejaculation at some time in their life. Premature ejaculation is one of the most common sexual problems. Premature ejaculation is explained as a condition where in a person is unable to delay ejaculation to a point when it is mutually desirable for both the partners. That means ejaculation occurs before a man wants it to happen. Some men ejaculate during foreplay while some do so at the very entry itself.

But few men feel that 5 or 10 minutes time to have orgasm is not enough and they feel that this is also premature ejaculation. This suggests premature ejaculation is most likely psychological in origin than physiological. The exact cause of premature ejaculation is still unknown.

Ayurveda has said Kshiprumunchati Shukram……. this means the person will have Premature ejaculation when vata in his body aggravates and leads to a very quick ejaculation of shukra (semen).

The probable causes for premature ejaculation:

• Anxiety during the first experience of sex. Some men will develop
a longer-term anxiety toward sex, which can cause a prolonged experience

• A long period of abstinence from sex.

• Younger men tend to ejaculate more quickly than older men, as
experience seems to be associated with ejaculatory control.

• Depression or anxiety about poor sexual performance

• Speak openly with your partners to avoid misunderstanding and

• Use some relaxation techniques to reduce stress.

• Have full knowledge about sex

• Try to minimize the above said causative factors.

Simple Remedies

Many techniques are used to control premature ejaculation. “The squeeze technique” is popular and effective amongst all. Squeeze technique is a behavioral therapy. If a man senses that he is about to experience premature ejaculation, he interrupts sexual relations. Then the man or his partner squeezes the shaft of his penis between a thumb and two fingers applying gentle pressure just below the head of the penis for 20 seconds. And then sexual relations can be resumed. The technique can be repeated as often as necessary. When this technique is successful, it enables the man to learn to delay ejaculation with the squeeze, and eventually, to gain control over ejaculation without the squeeze.

The Masters & Johnson method:

• The best way to combat premature ejaculation is by learning to
control the sensations prior to orgasm. This method takes time and
practice, but it is very effective.

• First you need to bring yourself close to orgasm (this can be
done via masturbation, without the involvement of your partner) and then
stop and relax before recommencing. Each time you need to bring yourself
closer to orgasm until finally you cannot control it. If you do this
often enough, you will learn where your point of climax is. This is
helpful when interacting with your partner.

• You will need to practice reaching your climax point with your
partner by engaging in non-penetrative sex so that when you feel it is
near, you signal them to stop and you allow your erection to subside.
This also needs to be repeated so that you and your partner are familiar
with the procedure.

• Once you feel you are ready for intercourse, it is best to start
by lying on your back so that you can guide your partner during
penetration. When you are near orgasm, give your partner a signal to stop
and you should relax and start again. Once you get the hang of it (it may
take several weeks or months), premature ejaculation shouldn’t be too
much of a problem.

• A variant of this method involves the partner squeezing the tip
of the penis just before orgasm (“squeeze technique”). This pushes blood
out of the penis and reduces the erection.

Viagra – How it Stimulates and Enhances Erections

Many men suffer from erectile dysfunction (ED) — the inability to achieve and sustain an erection long enough to have enjoyable and fulfilling sex. For a variety of reasons ED is most common in men over 45. The sex drive is often lowered even further in older men who are taking medications of various kinds.

Most of us assume the sex drive gradually diminishes as we get older, so we are prepared to accept ED as a natural condition. But the fact is, the inability for many men to sustain an erection may be more the result of treatable physical conditions rather than an actual loss of sex drive.

Until the early 1980s most people — including most trained urologists — assumed that ED was basically a mental or psychological problem. But in 1983 at a presentation to the American Urological Association, Dr. Giles Brindley demonstrated that a penis could be made erect by injecting it with the drug phentolamine. In a public demonstration, he used his own penis to make the point.

Brindley had discovered that the penis could be made erect by relaxing the normally consricted blood vessels leading into it. Once the blood vessels are relaxed, they let in more blood and the penis inflates something like a balloon to form an erection.

So gradually it came to be accepted that ED is the result of physical changes in the adult male — not primarily mental or psychological conditions.

Viagra – Controlling the Penile Valve

Phentolamine — the drug used by Dr. Brindley to give himself a public erection — started to be used with some success in the 80’s and 90’s, but there are two problems with phentolamine. First, it is not selective enough to target only the penis, so its effects on other parts of the body are unpredictable. And second, the erections it creates are not brought on by sexual stimulation. You can get an erection any old time with phentolamine, and you will continue to have one until the effect of the drug wears off.

Viagra was the first drug that overcame these two problems. (It was introduced in 1998 by the Pfizer company. The active ingredient in Viagra is sildenafil citrate.) Viagra does not work by relaxing the blood vessels of the penis. Instead it enhances the natural processes that take place when a man is sexually stimulated. It does this by controlling what we might call the “Softeners” — the chemicals in the body designed to make the penis go soft after an erection.

It works something like this. When a man is sexually stimulated his brain sends signals that trigger the production of chemicals that relax the blood vessels in the penis. Think of these as “Hardeners”. As the blood vessels are relaxed by the production of Hardeners, blood flow increases and the penis goes erect.

But nature has also designed a way for the erection to gradually subside. At the same time as Hardeners are being produced to relax the blood vessels, the body is also producing Softeners to break down and neutralize the Hardeners. These are an enzyme known as phosphodiesterase (PDE5).

As long as the sexually stimulated male is producing more Hardeners than Softeners, the penis will remain erect. But as soon as the production of Hardeners starts to go down, the Softeners will get the upper hand, and the erection will start to fizzle.

It is now generally assumed that ED is caused by an imbalance of Hardeners and Softeners. The body is either not producing enough Hardeners, or too many Softeners. Either way, the result is the inability to sustain an erection.

What Viagra does is block the production of Softeners. This allows the Hardeners to build up in the blood vessels of the penis and create hard, sustained erections.

The beauty of the process is that nothing happens without sexual stimulation. Viagra does not take the place of stimulation. Instead, it simply lets the natural chemicals created by sexual stimulation do their work. After sex, when the brain is no longer creating Hardeners, the erection subsides naturally. And when the effects of Viagra wear off — normally after 4 or 5 hours — the normal processes are restored to the way they were before taking the drug.

Of course, as with any drug, there are potential side effects. Viagra should not be taken if you are taking any nitrate-based drug, including nitroglycerin or if you are on any blood pressure or heart medication, as combining Viagra with these drugs can cause a severe drop in blood pressure. Also, if you have any liver or kidney problems it is not recommended that you take Viagra.
